Handgelenskorthese Manu Neurexa plus 28P30 und der Handlagerungsschiene 28P31.

3 Bestimmungsgemäße Verwendung

3.1 Verwendungszweck

Die Orthese ist ausschließlich zur orthetischen Versorgung des Handgelenks einzusetzen und

ausschließlich für den Kontakt mit intakter Haut bestimmt.

Die Orthese muss indikationsgerecht eingesetzt werden.

3.2 Indikationen

Parese der Unterarm- und Handmuskulatur

Gelähmte Unterarm- und Handmuskulatur mit leichter Spastik z.B. nach:

• Schlaganfall

• Verletzungen des Plexus brachialis

• Zervikaler Bandscheibenvorfall

Die Indikation wird vom Arzt gestellt.

3.3 Kontraindikationen

3.3.1 Absolute Kontraindikationen

• Mäßige und starke Spastiken der Unterarm- und Handmuskulatur

3.3.2 Relative Kontraindikationen

Bei nachfolgenden Indikationen ist eine Rücksprache mit dem Arzt erforderlich: Hauterkrankun

gen/-verletzungen, Entzündungen, aufgeworfene Narben mit Schwellung, Rötung und Überwär

mung im versorgten Körperabschnitt; Krampfadern stärkeren Ausmaßes, insbesondere mit Rück

fluss-Störungen, Lymphabfluss-Störungen – auch unklare Weichteilschwellungen körperfern des

Hilfsmittels; Empfindungs- und Durchblutungsstörungen im Bereich des Arms und der Hand.

3.4 Lebensdauer

28P30

Das Produkt ist für eine Lebensdauer von maximal 2 Jahren ausgelegt.

28P31

Das Produkt ist für eine Lebensdauer von maximal 1Jahr ausgelegt.

3.5 Wirkungsweise

Die Orthese stabilisiert das Handgelenk, wobei die Stellung des Handgelenks stufenlos einstell

bar ist und limitiert die Bewegung der Hand in palmarer oder dorsaler Richtung. Die Hand kann

wahlweise mit einer palmaren oder dorsalen Unterstützung gehalten werden. Die Orthese erleich

tert die therapeutische Behandlung, die Handfunktion und die körperliche Aktivität. Die Manu

Neurexa plus 28P30 ist kombinierbar mit der Handlagerungsschiene 28P31 und des Weiteren mit

der Schulterbandage Omo Neurexa plus 5065N.

4 Sicherheit

5 Handhabung

INFORMATION

►Die tägliche Tragedauer und der Anwendungszeitraum werden in der Regel vom Arzt festge

legt.

►Die erstmalige Anpassung und Anwendung des Produkts darf nur durch Fachpersonal erfol

gen.

►Weisen Sie den Patienten in die Handhabung und Pflege des Produkts ein.

►Weisen Sie den Patienten darauf hin, dass er umgehend einen Arzt aufsuchen soll, wenn er

außergewöhnliche Veränderungen an sich feststellt (z.B. Zunahme der Beschwerden).

5.1 Auswählen der Größe

Das Produkt wird in einer Universalgröße geliefert und ist beidseitig einsetzbar. Die Orthesenlän

ge und die Unterarmschalen lassen sich anpassen.

5.2 Anpassen

Orthesenlänge anpassen

Die Längeneinstellung der Orthese wird durch Verschieben der Unterarmschalen an der Arm

schiene erreicht. Die Armschiene kann an die Bedürfnisse des Patienten angepasst werden.

1) Die Schrauben der Schiene (siehe Abb.1, Pos. 4) mit dem Schlüssel (siehe Abb.1, Pos. 1)

lockern.

2) Den Ellbogen des Patienten in 90° Flexion bringen.

3) Die Orthese lateral am Unterarm anlegen.

4) Die Spangen der Orthese zwischen Handgelenk (siehe Abb.2) und ca. 2 cm vom Olecranon

platzieren (siehe Abb.3). INFORMATION: Die distale Spange ist auf dem Ulnarköpfchen

anzulegen. Sollte Patientenspezifisch eine Druckempfindlichkeit an dieser Stelle auf

treten, kann die Spange unmittelbar davor platziert werden. Es ist darauf zu achten,

dass der Rand der distalen Unterarmschale nicht in den Handrücken drückt.

5) Die Längeneinstellung auf der Schiene markieren.

6) Die Orthese abnehmen und die Schrauben mit dem Schlüssel anziehen (Anzugsmoment:

1Nm) (siehe Abb.4).

Winkeleinstellung der Orthese anpassen

Der einstellbare Winkel beträgt maximal 30° in Palmarflexion oder Dorsalflexion, dabei entspricht

eine Position jeweils 10°. Die Positionen sind durch Löcher im Orthesengelenk gekennzeichnet.

1) Die Schraube am Orthesengelenk mit dem Schlüssel lockern (siehe Abb.7).

2) Den Ellbogen des Patienten in 90° Flexion bringen.

3) Die Orthese lateral am Unterarm anlegen und ausrichten.

4) Das Orthesengelenk auf den gewünschten Winkel einstellen.

5) Die Position des Winkels markieren.

6) Die Orthese abnehmen und die Schraube mit dem Schlüssel anziehen (Anzugsmoment:

3,6Nm).

Handauflagenbügel anpassen

1) INFORMATION: Den Handauflagenbügel mit der Wölbung nach oben ausrichten.

Den Handauflagenbügel (siehe Abb.1, Pos. 3) in die Orthese (siehe Abb.8) einsetzen.

2) Den Ellbogen des Patienten in 90° Flexion bringen.

3) Die Orthese lateral am Unterarm anlegen.

4) Den Handauflagenbügel verschieben und entsprechend der individuellen Gegebenheiten un

ter der Handfläche positionieren.

5) Die Orthese abnehmen und die Schraube mit dem Schlüssel anziehen (Anzugsmoment:

1Nm).

Unterarmschalen anpassen

1) Den Ellbogen des Patienten in 90° Flexion bringen.

2) Die Orthese lateral am Unterarm anlegen.

3) Die benötigten Längen der Schalen markieren (siehe Abb.5).

4) Die Schalen mit einer Schere zuschneiden (siehe Abb.6).

5) Die Schnittkanten glattschleifen.

6) Den Sitz der Orthese nochmal überprüfen, wenn nötig nacharbeiten.

Unterarmpolster anpassen

>Voraussetzung: Die Orthese ist abgelegt.

1) Die selbstklebenden Klettpunkte innen auf die Schalen kleben (siehe Abb.9).

2) Die Polster anhand ihrer Form innen auf den Schalen platzieren und ankletten.

3) Die Polster mit 5 mm Überstand entlang der Schalenkanten mit einer Schere individuell kür

zen (siehe Abb.10).

Unterarmgurte anpassen

Die Unterarmgurte werden in den eingearbeiteten Schlitzen der Spangen befestigt. Bei korrekter

Montage muss sich die Zugschlaufe des Gurtes oben befinden.

1) Den Gurt durch den unteren Schlitz der Spange ziehen und am Klettverschluss schließen.

2) Den Gurt durch den oberen Schlitz der Spange ziehen und am Klettverschluss schließen, da

bei den Gurtumfang individuell am Arm anpassen.

Polster des Polstergurts für den Handauflagenbügel anpassen

>Voraussetzung: Die Orthese ist abgelegt.

1) Den Polstergurt (siehe Abb.1, Pos. 5) öffnen.

2) Das innenliegende Kunststoffröhrchen mit einer Schere auf Länge zuschneiden (siehe

Abb.11), dabei die Handbreite des Patienten als Maß berücksichtigen.

3) Optional: Den kürzeren gepolsterten Klettverschluss mit einer Schere kürzen (siehe

Abb.12).

4) Den Polstergurt über den Handauflagenbügel schieben und den Klettverschluss schließen

(siehe Abb.13, siehe Abb.14).

Gurt des Polstergurts für den Handauflagenbügel anpassen

>Voraussetzung: Die Orthese ist an den Unterarmgurten angelegt.

1) Die Hand auf den gepolsterten Handauflagebügel auflegen (siehe Abb.15).

2) Den Gurt über die Hand zwischen Daumen und Fingern ziehen und mit dem Klettverschluss

unter dem Polstergurt schließen (siehe Abb.16). INFORMATION: Patientenspezifisch den

Gurt unten oder oben schließen.

3) Optional: Den Klettverschluss mit einer Schere kürzen.

Handlagerungsschiene 28P31 anpassen

1) Die Hand des Patienten auf der Handauflage platzieren (siehe Abb.18, Pos. 5).

2) Die Daumenseite und die Fingergrößen auf der Handauflage markieren (siehe Abb.19).

3) Die Handauflage mit einer Schere entlang der Aussparungen auf die passende Größe zu

schneiden (siehe Abb.20).

4) Die Schnittkanten glattschleifen.

5) Den Daumen-Mittelhand-Verschlussgurt von unten beginnend einfädeln (siehe Abb.21), da

bei die Gurtumlenkschlaufe auf der Daumenseite platzieren. Die Gurtaussparung über den Si

cherungsverschluss für die Handauflage ziehen.

6) Den selbstklebenden Klettpunkt auf der Schiene im Bereich der Daumenmitte befestigen und

den Gurt daran fixieren (siehe Abb.22).

7) Den Verschlussgurt so kürzen, dass genügend Material für die Anbringung des Y-Klettsver

schlusses vorhanden ist. Den Y-Klettverschluss anbringen. Optional: Den Y-Klettverschluss

mit dem Gurt vernähen.

8) Den Finger-Verschlussgurt von unten beginnend in die Schiene einfädeln (siehe Abb.23,

siehe Abb.24, siehe Abb.25).

9) Die Handlage mit dem Handauflagenbügel verbinden (siehe Abb.26).

10) Den Sicherungsverschluss schließen (siehe Abb.27).

11) Die Hand des Patienten auf der Handlage positionieren.

12) Den zweigeteilten Verschlussgurt am Daumen schließen (siehe Abb.28).

13) Den zweigeteilten Verschlussgurt an der Mittelhand schließen.

14) Den Finger-Verschlussgurt schließen (siehe Abb.29).

Handauflagenpolster anbringen

1) Die selbstklebenden Klettpunkte gleichmäßig verteilt auf die Oberseite der Handauflage kle

ben.

2) Das Polster auf die Handauflage auflegen.

3) Die Größe des Polsters mit einer Schere zuschneiden.

5.3 Anlegen

Manu Neurexa plus 28P30 anlegen

>Voraussetzung: Alle Klettverschlüsse sind geöffnet.

>Voraussetzung: Die Orthese liegt nach oben geöffnet vor dem Patienten auf einem Tisch.

1) Den Unterarm in die Orthese einführen, bis die Hand komplett auf dem Handbügel aufliegt.

2) Die Gurte schließen.

3) Sicherstellen, dass die Gurte fest anliegen ohne abzuschnüren.

Manu Neurexa plus 28P30 mit Handlagerungsschiene 28P31 anlegen

>Voraussetzung: Alle Klettverschlüsse sind geöffnet.

>Voraussetzung: Die Orthese liegt nach oben geöffnet vor dem Patienten auf einem Tisch.

1) Den Unterarm in die Orthese einführen, bis die Hand komplett auf der Handlagerungsschiene

aufliegt.

2) Die Gurte schließen.

3) Sicherstellen, dass die Gurte fest anliegen ohne abzuschnüren.

Manu Neurexa plus 28P30 mit Omo Neurexa plus 5065N anlegen

>Voraussetzung: Die Abdeckungen der Druckknöpfe der Manu Neurexa plus 28P30 sind ent

fernt.

>Voraussetzung: Die Manu Neurexa plus 28P30 ist angelegt.

>Voraussetzung: Die Omo Neurexa plus 5065N ist angelegt.

>Voraussetzung: Die passenden Verbindungsgurte sind an den Druckknöpfen der Schulter

manschette befestigt.

>Voraussetzung: Die Unterarmmanschette der Omo Neurexa plus 5065N ist demontiert.

1) Die Klettverschlüsse der Verbindungsgurte öffnen.

2) Die Umlenkschlaufen an den Druckknöpfen der Handgelenkorthese befestigen (siehe

Abb.39, siehe Abb.40), dabei die gleichfarbigen Druckknöpfe miteinander verbinden

(schwarz-schwarz/rot-rot).

3) Die Verbindungsgurte von innen nach außen durch die Umlenkschlaufen führen und mit dem

Klettverschluss in der Länge einstellen (siehe Abb.41, siehe Abb.42). Dabei muss das

Schultergelenk entlastet und leicht nach außen rotiert sein.

4) Die Entlastungsfunktion an der Schulter prüfen. Wenn notwendig die Gurte in der Länge ver

stellen.

5.4 Umrüsten

Handauflage der Handlagerungsschiene 28P31 auf den Handauflagenbügel der Manu

Neurexa plus 28P30 umrüsten

1) Alle Gurte öffnen (siehe Abb.30, siehe Abb.31).

2) Den Sicherungsverschluss öffnen (siehe Abb.32).

3) Die Handauflage vom Handauflagenbügel abnehmen (siehe Abb.33, siehe Abb.34).

4) Das Handauflagenpolster mit Fixiergurt auf dem Handauflagenbügel anbringen (siehe

Abb.35, siehe Abb.36) und den Gurt verschließen (siehe Abb.37).

5.5 Ablegen

1) Alle Gurte öffnen.

2) Die Orthese vom Unterarm abnehmen.

6 Reinigung

HINWEIS

Verwendung falscher Reinigungsmittel

Beschädigung des Produktes durch falsche Reinigungsmittel

►Reinigen Sie das Produkt nur mit den zugelassenen Reinigungsmitteln.

Textilpolster

1) Alle Textilpolster von der Orthese entfernen.

2) Alle Klettverschlüsse schließen.

3) Empfehlung: Waschbeutel oder -netz verwenden.

4) Die Textilpolster in 30°C warmen Wasser mit einem handelsüblichen Feinwaschmittel wa

schen. Keinen Weichspüler verwenden. Gut ausspülen.

5) An der Luft trocknen lassen. Direkte Hitzeeinwirkung vermeiden (z.B. durch Sonnenbestrah

lung, Ofen- oder Heizkörperhitze).

6) Die Textilpolster wieder an der Orthese anbringen.

Orthesenrahmen

1) Bei Bedarf mit einem feuchten Lappen abwischen.

2) An der Luft trocknen lassen. Direkte Hitzeeinwirkung vermeiden (z.B. durch Sonnenbestrah

lung, Ofen- oder Heizkörperhitze).

7 Entsorgung

Das Produkt gemäß den geltenden nationalen Vorschriften entsorgen.

8 Rechtliche Hinweise

Alle rechtlichen Bedingungen unterliegen dem jeweiligen Landesrecht des Verwenderlandes und

können dementsprechend variieren.

8.1 Haftung

Der Hersteller haftet, wenn das Produkt gemäß den Beschreibungen und Anweisungen in diesem

These instructions for use provide important information on the fitting and application of the

28P30 Manu Neurexa plus wrist orthosis and the 28P31 wrist positioning splint.

2 Product description

2.1 Product overview

3 Intended use

3.1 Indications for use

The orthosis is intended exclusively for orthotic fittings of the wrist joint and exclusively for con

tact with intact skin.

The orthosis must be used in accordance with the indications.

3.2 Indications

Pareses of the forearm and hand musculature

Paralysed forearm and hand musculature with slight spasticity, forinstance after:

• Stroke

• Brachial plexus injuries

• Cervical intervertebral disc prolapse

Indications must be determined by the physician.

3.3 Contraindications

3.3.1 Absolute Contraindications

• Moderate and severe spasticity of the forearm and hand musculature

3.3.2 Relative Contraindications

The following indications require consultation with a physician: skin diseases/injuries, inflamma

tion, prominent scars that are swollen, reddening and hyperthermia of the fitted limb; pronounced

varicose veins, especially with return flow impairment, lymphatic flow disorders, including unclear

soft tissue swelling distal to the support; sensory and circulatory disorders in the area of the arm

and hand.

3.4 Lifetime

28P30

The product is designed for a maximum lifetime of 2 years.

28P31

The product is designed for a maximum lifetime of 1year.

3.5 Effects

The orthosis stabilises the wrist joint; the position of the wrist joint is continuously variable, and

the movement of the hand is limited in the palmar or dorsal direction. The hand can be held with

either palmar or dorsal support. The orthosis facilitates therapeutic treatment, hand function and

physical activity. The 28P30 Manu Neurexa plus can be combined with the 28P31 hand position

ing splint and also the 5065N Omo Neurexa plus shoulder support.

4 Safety

5 Handling

INFORMATION

►The daily duration of use and period of application are generally determined by the physi

cian.

►The initial fitting and application of the product must be carried out by qualified personnel.

►Instruct the patient in the handling and care of the product.

►Instruct the patient to see a physician immediately if any exceptional changes are noted (e.g.

worsening of the complaint).

5.1 Size Selection

The product is supplied in a universal size and can be used on both sides. The orthosis length

and forearm supports can be adapted.

5.2 Adaptation

Adapting the orthosis length

The length of the orthosis is changed by sliding the forearm supports on the arm bar. The arm bar

can be adapted to the requirements of the patient.

1) Loosen the screws for the bar (see fig.1, item 4) with the wrench (see fig.1, item 1).

2) Place the patient's elbow in a 90° flexion position.

3) Apply the orthosis laterally on the forearm.

4) Position the supports of the orthosis between the wrist joint (see fig.2) and approx. 2 cm from

the olecranon (see fig.3). INFORMATION: The distal support has to be applied on the

ulnar head. In case of patient-specific pressure sensitivity at this point, the support

can be applied directly before it. Ensure that the edge of the distal forearm support

does not press on the back of the hand.

5) Mark the length adjustment on the bar.

6) Take off the orthosis and tighten the screws with the wrench (torque value: 1Nm) (see fig.4).

Adapting the orthosis angle

The angle adjustment range is 30° max. in palmar flexion or dorsiflexion, with one position

respectively corresponding to 10°. The positions are marked by holes in the orthosis joint.

1) Loosen the screw on the orthosis joint with the wrench (see fig.7).

2) Place the patient's elbow in a 90° flexion position.

3) Apply the orthosis laterally on the forearm and align it.

4) Set the orthosis joint to the desired angle.

5) Mark the angle position.

6) Remove the orthosis and tighten the screw with the wrench (torque value: 3.6Nm).

Adapting the hand support bar

1) INFORMATION: Align the hand support bar with the curve facing up.

Insert the hand support bar (see fig.1, item 3) into the orthosis (see fig.8).

2) Place the patient's elbow in a 90° flexion position.

3) Apply the orthosis laterally on the forearm.

4) Slide the hand support bar and position it under the palm according to the individual condi

tions.

5) Take off the orthosis and tighten the screw with the wrench (torque value: 1Nm).

Adapting the forearm supports

1) Place the patient's elbow in a 90° flexion position.

2) Apply the orthosis laterally on the forearm.

3) Mark the required lengths of the supports (see fig.5).

4) Trim the supports with scissors (see fig.6).

5) Sand the cut edges smooth.

6) Check the fit of the orthosis again and rework it as needed.

Adapting the forearm pad

>Prerequisite: Take off the orthosis.

1) Apply the self-adhesive hook-and-loop patches to the inside of the supports (see fig.9).

2) Position the pads inside the supports according to their shape and fasten with hook-and-loop.

3) Individually trim the pads with scissors, leaving a 5 mm overhang along the edges of the sup

ports (see fig.10).

Adapting the forearm straps

The forearm straps are attached in the integrated slots in the supports. With correct assembly,

the pull loop of the strap has to be on top.

1) Pull the strap through the lower slot in the support and fasten it with the hook-and-loop clos

ure.

2) Pull the strap through the upper slot in the support and fasten it at the hook-and-loop closure,

individually adapting the strap circumference to the arm.

Adapting the pad of the padded strap for the hand support bar

>Prerequisite: Take off the orthosis.

1) Open the padded strap (see fig.1, item 5).

2) Cut the interior plastic tube to length with scissors (see fig.11), taking the patient's hand

width into account as the measurement.

3) Optional: Trim the shorter padded hook-and-loop closure with scissors (see fig.12).

4) Slide the padded strap over the hand support bar and fasten the hook-and-loop closure (see

fig.13, see fig.14).

Adapting the padded strap for the hand support bar

>Prerequisite: The orthosis is attached to the forearm straps.

1) Lay the hand onto the padded hand support bar (see fig.15).

2) Pull the strap over the hand between the thumb and fingers, and fasten it with the hook-and-

loop closure under the padded strap (see fig.16). INFORMATION: Close the strap at the

bottom or top, depending on the patient.

3) Optional: Shorten the hook-and-loop with scissors.

Adapting the 28P31 hand positioning splint

1) Position the patient's hand on the hand support (see fig.18, item 5).

2) Mark the thumb side and the finger sizes on the hand support (see fig.19).

3) Trim the hand support to the correct size along the slots using scissors (see fig.20).

4) Sand the cut edges smooth.

5) Starting on the bottom, thread the thumb-metacarpus closure strap (see fig.21), positioning

the strap guide loop on the thumb side. Slide the strap opening over the safety closure for the

hand support.

6) Attach the self-adhesive hook-and-loop patch to the bar in the area of the centre of the thumb

and use it to secure the strap (see fig.22).

7) Shorten the closure strap so that there is sufficient material for attaching the Y-hook-and-loop

closure. Attach the Y hook-and-loop closure. Optional: Sew the Y-hook-and-loop closure to

the strap.

8) Starting on the bottom, thread the finger closure strap into the bar (see fig.23, see fig.24,

see fig.25).

9) Connect the hand support to the hand support bar (see fig.26).

10) Fasten the safety closure (see fig.27).

11) Position the patient's hand on the hand support.

12) Fasten the split closure strap on the thumb (see fig.28).

13) Fasten the split closure strap on the metacarpus.

14) Fasten the finger closure strap (see fig.29).

Applying the hand support pad

1) Attach the self-adhesive hook-and-loop patches evenly distributed to the top of the hand sup

port.

2) Put the pad onto the hand support.

3) Use scissors to cut the pad to size.

5.3 Application

Application of the 28P30 Manu Neurexa plus

>Prerequisite: All hook-and-loop closures are open.

>Prerequisite: Place the orthosis on a table in front of the patient, opened to the top.

1) Insert the forearm into the orthosis until the hand is lying fully on the hand support.
